INDIVIDUAL APPLICATION PROCESS

All individual applications MUST be completed using the CPD Application Form following a discussion and agreement with your line manager about service needs and your development needs at your apprainsal or 1-1 meeting.

The application form MUST be signed and agreed by your line manager with final approval given by your Head of Nursing or CBU ADO. All CPD applications are recorded centrally for monitoring purposes.

For CBU Staff
If you work in Specialist Services, Planned Care or Urgent Care, your application will be processed within your CBU once signed off by the Head of Nursing.

For Corporate Nurses, AHPs or Midwives please send your completed and
fully signed applications forms to the following email address for processing:
soh-tr.cpdapplications@nhs.net

GROUP APplication process

In some cases, CPD opportunities may be available to groups of staff and the application for CPD funding may differ to the previous process.

In this instance, a ‘CPD Application Process for external programme(s) for large groups’ form will need to be completed by the departmental manager, on behalf of the group.

The details of the individual staff members will be entered on one form, to reduce the likelihood of multiple forms becoming misplaced.

This form will then require final approval and sign off from the Head of Nursing of your CBU, and the administrative process to release funding will commence.
