Thank you for completing the Serious Incident (SI) experience and management questionnaire.
I am an ST6 in Respiratory Medicine currently working as a Medical Education Fellow for North West London Local office. One of my tasks is to evaluate the SI management process and subsequent experience across Health Education England (HEE). This project aims to help generate a more standardised and transparent approach for SI management where trainees are involved with an emphasis on maximising education and training opportunities. We know that currently there is often inconsistency generated by variation in local policies, local interpretations, perceptions of what constitutes an SI and what constitutes appropriate methods of escalation.
Through this questionnaire we hope to gain insight into current experiences of postgraduate medical trainees in adverse event management across England. The vision is to use feedback from questionnaires circulated across trainees and trainers from all of HEE’s 13 local offices to gauge similarity and diversity in actual experience, for both the trainee and their trainer.
The greater the number of responders the more useful the feedback and as such your participation is greatly appreciated. Any feedback will be anonymised aside which area the feedback was received from.
